Penumbra Reaches New Heights with Ruby® XL, Advancing Precision in Vascular Embolization

The launch of Ruby® XL represents a breakthrough in vascular embolization, offering physicians unprecedented flexibility and efficiency for complex case management.

In an exclusive conversation with MedTech Spectrum, Ms. Shruthi Narayan, President, Interventional at Penumbra, discusses how the Ruby® XL System is setting a new benchmark in vascular embolization. Engineered to be the world’s longest, largest, and softest coil, Ruby® XL is designed to enhance control, precision, and efficiency in treating complex, high-volume cases. With its unmatched performance characteristics, the system empowers physicians to achieve faster occlusion and reduce procedural complexity. As Penumbra continues to lead innovation in interventional care, Ruby® XL reinforces its commitment to delivering transformative technologies that improve both clinician workflow and patient outcomes.

What inspired the development of the Ruby® XL System, and how does it address unmet needs in complex embolization procedures?

The inspiration for Ruby XL came from physician feedback and the need for a larger, longer, softer coil compatible with diagnostic catheters. The Ruby XL System is designed to provide what physicians have been asking for: a coil platform with more volume and fewer exchanges that can be delivered without a microcatheter. The system addresses a lot of the limitations of existing .035 coils, delivering mechanical occlusion through more volume without sacrificing softness or deliverability.

How do the design features—being the longest, largest, and softest coil—translate into clinical and procedural benefits for interventional radiologists?

Ruby XL System is a new class of coil that offers the longest coil on the market today at 70cm and the largest size at 40mm. It also offers the largest primary diameter on the market at .030 inches. These three attributes together can simplify embolization procedures, allowing physicians to complete these procedures with fewer coils. The system offers up to 3x more volume than other 35-system coils available today on the market.

Could you elaborate on how the Ruby XL, POD XL, and Packing Coil XL work together to improve procedural efficiency and precision?

Each of these new technologies offers a feature that allows them to work together. Ruby XL is designed to frame large aneurysm sacs of up to 40mm. POD XL offers a unique 3-in-1 occlusion technology that allows it to anchor, frame and pack in one coil in high flow vessels. Packing Coil XL features an innovative “liquid metal” wave shape technology that allows it to conform to any vessel diameter. Its unique softness profile makes it ideal as an effective secondary coil in these clinical situations.

What role does this new system play in expanding treatment possibilities for large vessel or high-flow embolization?

The Ruby XL System offers a technology that provides a significant volume advantage over other existing .035 coils on the market. POD XL has been specifically designed to address high-flow clinical situations. With the Ruby XL System, physicians can effectively address embolization procedures such as gastric varices, pelvic venous disease, and endoleaks.

How does the Ruby® XL System reflect Penumbra’s broader strategy in advancing peripheral embolization and improving patient outcomes?

The launch of Ruby XL embodies Penumbra’s commitment to innovation across a variety of medical conditions. Penumbra's Ruby portfolio of peripheral embolization products, now with the Ruby XL System, provides physicians with one of the most expansive embolization offerings, enabling physicians to have the latest coil technologies and most extensive options for every case.
